Individualized Student Planning and Support

Even the strongest systems can struggle to support students whose needs fall outside traditional models. These are often the most complex situations—where disability, environment, behavior, and learning profiles intersect in ways that challenge existing programs. This is also where thoughtful, individualized planning makes all the difference.

When teams feel stuck or overwhelmed, we offer a path forward. Our goal is to help schools and families develop creative, research-informed solutions that allow students to remain in district, surrounded by familiar peers and staff. Through targeted assessments, collaborative planning, and responsive supports, we work alongside IEP teams to build individualized plans that truly work.

Supporting Teams to Support Students

We support teams in designing student-centered services that blend evidence-based practices with creativity and flexibility. This includes both academic and functional planning for students with complex profiles—those whose needs require thoughtful adjustments outside the typical continuum.

Services may include:

  • Developing specially designed instructional plans aligned with IEP goals

  • Creating individualized inclusion plans with appropriate supports

  • Designing classroom systems and visual supports

  • Conducting secondary transition assessments and planning

  • Evaluating assistive technology needs and making recommendations

  • Reviewing existing documentation and offering clear, actionable recommendations

  • Participating in IEP meetings to support team planning and problem-solving

Whether you're navigating a tough decision, planning for transition, or creating new supports from the ground up, we can help your team move forward with clarity and confidence.
